Great place for rehab. Staff is wonderful and attentive to my needs. I have been to quite a few of these places over the years and this one is at the top of my list.

So very impressed with the staff at this facility. Met with her care team today and I found them to be very caring and committed to providing her with the very best care possible. Thank you Robert, Nutrition Director, Joseph, Activities Director, Erwin, Rehabilitation Director, Mark, Physical Therapy, Nurses Casey and Lisa. Horrible experience. My mom died there and no one on the staff ever called me. I found out from her neighbor that she had past 4 hours earlier. Furthermore, to this day (4 months later) I still do not know the details leading up to her death. The administrator never returned my calls. I had to make a formal report against this institution.

Am I thankful I found this place! Unfortunately, my 80 year old grandmother has been in and out of nursing homes for about 6 years now. Every time she goes somewhere I can't help but wonder what is going on. She has had 3 knee surgeries and then a couple knee complications after being sent home from awful places. My grandmother lives in phoenix and has gone to places located close to home but this time around we toured places and asked a lot of questions. When I toured this location I fell in love with what I saw and the people I had a chance to interact with during the tour. I spoke to the therapy guy who told me he believes they have a wonderful therapy program and have had amazing results. My grandmother was at South Mountain for roughly 3 weeks receiving therapy 6 days a week and to my surprise she was quite delighted by her overall experience. The whole staff was very friendly and attentive to my grandmother. She is thankfully now home and says she was very pleased with her experience there. Although, ideally she never wants to return to a nursing home (nor do I wish for her to return any place) I know if she does my top choice will be South Mountain! Thank you all so much!

they did not change my grandmothers life vest battery and no one knows how long she was like that and she ended up in the hospital, they were suppose to change her wound every time she went to the bathroom and they were never on the same page, the nurse was rude when we asked about it with no compassion. I would not advise you to take anyone here. THIS PLACE SHOULD NOT GET ANY STARS
